Every medicine has both generic and brand names!
-Generic name – lower case, only one; every medicine has a generic name, even if it is new and under patent and there is only one Brand Name version available at this time
-Brand Name© – upper case, maybe italicized; there could be many
-Chef’s choice: generic/Brand Name©

-What: include every medicine, including supplements, vitamins, creams, etc. -include Sig (signetur, latin for let it be labeled) amount-route-frequency
-Why : the reason for taking or stopping it
-When, -Who, Why: start and stop dates, which medical provider started and stopped Storing meds -Label caps
-What to do if Sig changes from what pharmacy wrote
